This policy covers modwella.com and the forms on it. When you book or receive care, your medical information is also protected under our Notice of Privacy Practices, which controls if the two differ.
What we collect
- What you send us. Your name, phone, email and question when you use our contact form. Your answers when you take a questionnaire, such as the women’s hormone quiz.
- Booking. When you tap Book, you leave this site for our booking system, run by Zenoti. Zenoti collects what you enter there for us.
- Texts and calls. Messages you send to our nurses and dispatchers.
- Site use, only if you agree. If you tap Accept on our cookie banner, we use Google Analytics (through Google Tag Manager), Microsoft Clarity and, on some pages, the Meta Pixel to see how the site is used and to measure our ads.
Health information and advertising
- We never sell your information.
- We do not send your questionnaire answers, contact form messages or the service you are interested in to any advertising company.
- The Meta Pixel never loads on our health service pages (hormones, weight loss, labs, iron, regenerative medicine, pregnancy and similar) or on any questionnaire page.
- Our questionnaire pages load no analytics or advertising tools at all.
How we use it
- To answer your questions, book and provide your care, and follow up.
- To text or call you, if you agreed to that. See our SMS Terms.
- To improve the site and measure our ads, only if you accepted cookies.
- To meet legal and safety duties.
Who we share it with
- Service providers who help us run Modwella, such as our booking system (Zenoti), form and email tools [list each, and confirm which have signed a HIPAA business associate agreement].
- Your care team: our nurses and providers.
- When the law requires it.
Your choices
- Cookies. Tracking stays off unless you tap Accept. Change your mind at any time with “Cookie settings” in the footer. We treat a Global Privacy Control signal from your browser as a “no”.
- Texts. Reply STOP to any text to opt out.
- Your data. Ask to see, correct or delete the information we hold about you by contacting us below. We answer within the time your state’s law requires.
Consumer health data (Washington, Nevada and similar laws)
This section is our consumer health data privacy policy. The consumer health data we collect on this site is what you choose to send us: questionnaire answers and anything you write in a message. We use it only to respond to you and provide care. We collect it only with your consent, we do not sell it, and we share it only with the service providers listed above. You may ask to see it, delete it, or withdraw your consent at any time by contacting us below. If we deny a request, you may appeal by replying to our answer; if we deny the appeal, you may contact your state Attorney General.
California residents
You have the right to know what personal information we collect, to delete it, to correct it, and to opt out of its sale or sharing for cross-context advertising. We do not sell personal information. Declining cookies, or sending a Global Privacy Control signal, opts you out of sharing for advertising. We will not treat you differently for using these rights.
Security
We use encryption in transit on this site and limit who can see your information. No system is perfectly secure.
Children
This site is not for children under 13, and our questionnaires are for adults 18 and older.

We will post any change here and update the date at the top.
